PATIENT REGISTRATION

 

For the team at TOMIS to provide the best care for you, or your family, we require comprehensive details of your medical history. This will enable us to treat you with optimal and personalised care.

INITIAL APPOINTMENT

 

Your initial appointment with Dr.Garg will involve a comprehensive consultation that will involve an examination , explaining your diagnosis and treatment options.

At your first visit, we will need you to complete some forms and thus request you to arrive at least 10 minutes prior to your appointment time.

Required Information

Please assist us by proving the following info at the time of your consultation:

  • Referral document (must be current)

  • X-rays (if applicable)

  • Health Insurance and Medicare details

  • Current list of your medications

  • Next of kin contact information

Other Considerations

Please make arrangements to have all your documents available at the time of your consultation.

Please bring your private health insurance card, if available.

Please alert our office and your surgeon if you have a medical condition that may impact your surgery (diabetes, high blood pressure, artificial heart valves and joints, osteoporosis, rheumatic fever, etc.) or if you are taking any medications.

 

PATIENT ASSESSMENT

 

Assessing the eligibility of the patients is the responsibility of the surgeon, and the anaesthesiologist.

While this may vary, some of the key criteria to assess your suitability for surgery includes:

  • Age

  • General Health

  • Type and length of procedure

  • Access to post operative care

Completing our patient assessment form at the time of your initial consult will allow us to provide you with the highest standard of care.

If you have any concerns about your treatment plan or diagnosis, please ask your surgeon or the anaesthetist.

HOSPITAL PAPERWORK

 

Depending on the location of your surgery, each hospital requires different paperwork to be completed, prior to your admission. Please ask our staff prior to your surgery to ensure all documentation is up to date, to ensure smooth admission and discharge for your surgery.

 

BEFORE GENERAL ANAESTHESIA

 

Undergoing surgery can be a challenging and unfamiliar experience for many patients. For your best recovery, it is crucial that your are mentally and physically prepared as possible.

FASTING

It is imperative that no foods or liquids are consumed for at least six hours prior to your surgical appointment.

MEETING YOUR ANAESTHETIST

Before undergoing surgery, you will have a brief consult with your anaesthetist before entering the operating room. They will talk to you about the type of anaesthetic they will use and address any specific concerns you may have.

OVERALL HEALTH

You should be as healthy as possible before undergoing surgery or anaesthetic - if you are feeling unwell, please tell your anaesthetist or the surgeon immediately.

 

SURGERY CHECKLIST

 
  • No food or liquids (including water) for six hours prior to the surgery.

  • No smoking at least 12 hours before surgery. Ideally, cut down or stop smoking as soon as possible prior to the day of surgery.

  • A responsible adult must accompany the patient to the hospital, and drive the patient home.

  • The patient is not to drive a vehicle or operate any machinery for 24 hours following general anaesthesia.

  • Wear comfortable clothing to the hospital.

  • Contact lenses, jewellery, and dentures must be removed at the time of surgery.

  • Do not wear lipstick, excessive makeup, or nail polish on the day of surgery.

  • If you have an illness such as a cold, sore throat, stomach or bowel upset, please notify the office.

  • If you take routine oral medications, please check with your doctor prior to your surgical date for instructions.
